Office Policies and Hours:

You, as a patient, come first in our medical practice. We strive to provide the best care possible, and a lot of work goes on behind the scenes to provide you with the best care possible. Please read below to learn about our policies and the hours we are open.

Office Hours:

Our practice is open Monday to Thursday 8AM-5PM (closed 12-1PM for lunch). We are closed for national holidays.

Scheduling an Appointment:

When calling to schedule a new patient appointment, you will need the following: Patient name, date of birth, patient phone number, insurance provider, insurance identification number and group number (both are on your insurance card), name of the insured person. If you have an HMO plan or one that requires a referral to see a specialist, the referral must be in the office prior to scheduling the appointment. Please have your primary care doctor fax the referral to the office.

What To Bring to Your Appointment:

Please bring your photo identification (driver’s license, photo ID card, passport), drivers license number, and insurance card.

New Patient Paperwork:

Please print out the new patient paperwork and bring it with you to your appointment. You may download the new patient paperwork here: PDF. If you are unable to complete the paperwork prior to your appointment, we will give you this paperwork to complete when you arrive. Please arrive 15 minutes prior to your appointment time so that we may input all of your forms and information into our computer system. We will make every effort to accommodate late arrivals, but it may be necessary to reschedule those arriving late in order to try to keep our other patients appointments on time.

Cancelling an Appointment:

If you find that you need to cancel or reschedule an appointment PLEASE call us 24 hours in advance so that we may fill that time with another patient who needs to be seen. For patients who do not show up for the appointment they scheduled and do not call to cancel the appointment, there will be a $40 charge, since that appointment slot was being held for you and others had been turned away. For appointments cancelled with less than a 24 hour notice, there will be a $40 charge to you if we are unable to fill the appointment slot. If you gave us enough notice so that we can fill that slot that you had reserved, then there will not be a charge. We understand that emergencies and illnesses come up, and this will be taken into account and not charged for unless it happens repeatedly. Cancellations in the evenings or on the weekends can be left on the voicemail. We have a computer system that will call you at least 2 days before your appointment, at the phone number you gave us, to confirm your appointment. This confirmation call should give you enough time for you to call with a 24 hour notice if you need to cancel.

Messages/Phone Calls:

We realize the importance of your calls and we strive to answer them in a timely manner. When calling to leave a message, please: state your full name and SPELL your last name, give your date of birth, phone number, and reason for your call. We occasionally get messages without all the information needed to be able to look up your medical record or return your call. Please call us back if you do not hear from us. Please allow 24-48 hours during the work week for us to return non-emergent phone calls. For emergencies, please call 911 or go to your nearest emergency room.

Prescription Refills:

Sometimes the doctor is able to give refills without seeing a patient first, but other times the patient must be seen before further refills can be given. Please keep in mind that some conditions do need to be monitored by the doctor, and for that reason you will be asked to follow-up in the office prior to refills being given. One year after prescriptions are written, they expire, and the patient must be seen for a refill.

Medical Advice:

Only the doctor and the nurse are able to give medical advice and treatment recommendations, so please do not expect that the receptionists can help you when you call to schedule an appointment. With skin conditions, the doctor must often look at the problem to figure out what treatment is best. We look forward to seeing you at your appointment.

We accept cash, checks, and credit cards (Mastercard, Visa, Discover, American Express, and Care Credit).
